2. Copy of the applicant's ID (signed)
3. Copy of the guarantor's ID (signed)
4. Passport-sized photo for membership card
5. Academic certificate (for UBD Alumni only)
6. Letter of employment or any proof of employment (for civil servant and uniformed personnel only)

Incomplete application form and/or incomplete documents and payment shall not be processed.
